Clinical Pharmacist

Our team of Clinical Pharmacist work as part of the Southampton West PCN practice teams and work as part of a multi-disciplinary team using there expert knowledge of medicines to support long term condition management. Our Clinical Pharmacist are able to support patient care with chronic diseases and undertake clinical medication reviews. Our Clinical Pharmacist Team is essential and provide high quality care across the PCN.

A Clinical Pharmacist can:

  • Work closely with GPs to discuss patient care
  • Work directly with patients and review medication
  • Manage long term conditions
  • Support Pharmacy Technicians with medication related queries
  • Complete medication audits to ensure patient safety
  • Participate in PCN or practice MDTs

Meet The Team


Enhanced Access

Southampton West PCN have partnered with Push Dr to deliver Enhanced Access appointments. Push Dr deliver online consultations for patients registered within Southampton West PCN member practices between 6.30pm – 8pm Monday – Friday and 9am – 5pm Saturday. Clinicians offering online consultations can support patients with medical advice, referral letters, and sick notes for many conditions.

First Contact Physiotherapy

Our team of First Contact Physiotherapists work across our GP practices to provide a specialist MSK service offering expert assessment, advice, guidance, and management of musculoskeletal conditions. Our team can direct patients for treatment and/or rehabilitation, when necessary, within the Solent MSK pathway or other relevant services.

A First Contact Physiotherapist (FCP) can:

  • Provide direct access to expertise in a Primary Care setting
  • Conduct a comprehensive MSK assessment
  • Support patients with treatment plans
  • Asses need for other services following initial assessment

Health & Wellbeing Coach

Our team of Health & Wellbeing Coaches work collaboratively within Southampton West PCN to help encourage sustainable lifestyle change within the community, the team support patients to make positive lifestyle changes that will help patients become the healthiest and happiest versions of themselves.
The Health & Wellbeing Coach Service offered by Southampton West PCN aims to create lasting change to improve patients' wellbeing and overall health. The team are driven to motivate people to feel more confident in taking pride and ownership in managing their own health and wellbeing

The team support patients with conditions such as;

  • Type 2 diabetes
  • Prediabetes
  • PCOS, obesity and difficulties with fertility
  • Metabolic syndrome
  • Hypertension
  • Sleep Apnoea
  • BMI > 30

Mental Health Practitioners

Our team of Mental Health Practitioners are committed to providing comprehensive and accessible mental health support within the primary care setting. Our team serves as a direct point of contact for patients seeking mental health assistance, reducing barriers to access and ensuring timely support.

A Mental Health Practitioner can:

  • Provide a holistic assessment to identify patient needs
  • Support patients with brief Interventions
  • Identify a need for onward referral
  • Help identify and signpost to Community groups and/or events

Pharmacy Technician

Our team of Pharmacy Technicians work alongside our core practice teams within Southampton West PCN member practices. We have partnered with Core Prescribing Solutions to deliver a seamless service for our patients. Our Pharmacy Technicians work closely with our expert team of Clinical Pharmacists as well as with the practice team of GPs, Nurses, Administrative staff. 

A Pharmacy Technician can:

  • Respond to acute medication requests
  • Manage medication queries
  • Liaise with local community Pharmacies and other care providers
  • Reconcile medications from hospital discharges
  • Review clinical audits

Social Prescriber

Our team of Social Prescribers work alongside our patients and practice teams by offering support that extends beyond medical treatment. Our dedicated team link with many local services and agencies in order to be able to support our patients in the best way possible.

A Social Prescriber can support with:

  • Lifestyle changes
  • Social Support
  • Education and Skills
  • Volunteering
  • Housing
  • Money Worries
  • Bereavement
  • Loneliness and Isolation
  • Mental health & Wellbeing
