As the Frailty Lead, Dr Emma Hilton coordinates and clinically leads the multi-disciplinary team (MDT) meeting with the PCN, local care homes, and community providers. Dr Emma Hilton works with community providers, care homes, and local systems to support positive pathway changes to ensure the best possible care for patients living in care homes.
